If you see blood in the vomit or poop, don’t Google it. Just go. A little streak of red from forceful vomiting is sometimes okay (think of a tiny broken blood vessel), but a lot of red or a “coffee grounds” look means internal bleeding. That’s an immediate “bye-bye, let’s go” situation.
What about bright green or yellow vomit? That’s bile, and it can mean their intestines are blocked or twisted. This is rare, but it’s not a “wait and see” scenario. You also need to worry if your child is vomiting for more than 24 hours without keeping anything down—not even a sip of water. Their body is in full revolt.
Oh, and a fever that hits 104°F (40°C) or higher? That’s not just a “high fever.” That’s a fever having a party in your kid’s body, and you need to crash it with a doctor’s help. Especially in babies under three months—any fever over 100.4°F is an automatic ER visit. No exceptions, no “let’s see how they feel in the morning.”

Kids have a terrible sense of location when it comes to pain. They’ll say their tummy hurts when it’s actually their throat. But persistent, sharp pain that makes them curl up into a fetal position and scream? That’s not gas. That could be appendicitis or a blockage.
Watch for them walking hunched over, or if they can’t jump. If the pain is localized to the lower right side, that’s a classic appendix trick. Also, if they cry when you gently press on their belly and then quickly release your hand (that’s called “rebound tenderness”), don’t wait. Call your doctor.
And here’s a weird one: if they vomit but still have a huge poop stuck in the toilet? Or if the vomiting is projectile, like a fire hose aimed across the room? That’s a sign of a bowel obstruction in some cases. It sounds silly, but it’s a real thing.
